What this template covers

The template follows the structure set out in ACOP L8 and HSG274 Part 2. There is no official HSE legionella risk assessment template, despite how often people search for one, so a structured template that follows the HSE's own guidance is the practical alternative. It includes sections for:

  1. Premises details. Address, duty holder, responsible person, date of assessment, date of next review.
  2. Water system description. Mains supply, storage tanks, calorifiers, distribution, outlets. Space for a schematic or annotated drawing.
  3. Risk identification. Temperature control, stagnation, little-used outlets, dead legs, TMVs, shower heads, and any higher-risk features.
  4. People at risk. Employees, tenants, visitors, and anyone with increased susceptibility.
  5. Written scheme of control. Tasks, frequencies, and responsible individuals for monitoring, flushing, cleaning, and sampling.
  6. Records. A template monitoring log for temperature readings and flushing records.
  7. Review. A log for scheduled reviews and any interim reviews triggered by changes to the system.

Who this template is for

This free template is designed for premises with a simple water system: single domestic lets, small offices, shops, holiday lets without hot tubs, and small commercial premises with a combi boiler or a single hot water cylinder. It is a reasonable starting point for anyone who needs to document a legionella risk assessment for this type of building.

For more complex premises (care homes, hospitals, leisure facilities, premises with cooling towers, any healthcare setting), a generic template is not appropriate. These need a bespoke assessment from a competent assessor, usually one working for a Legionella Control Association registered company. Is a template enough?

A template gives you the structure. It does not give you the competence to identify every risk or design a scheme that actually works for your specific system. For a duty holder who understands their water system well and can apply the guidance in HSG274 Part 2, a template is a reasonable starting point. For anyone unfamiliar with the regulatory framework, the template is a useful reference but should not be used as a fill-in-the-blanks exercise without reading the underlying guidance.
